Your Feelings Over the Past 4 Weeks
Thank you for taking part. These five questions ask how you have been feeling recently, and your answers help us understand mental wellbeing and plan support.
These questions are the Mental Health Inventory-5 (MHI-5), the emotional well-being items of the RAND 36-Item Health Survey 1.0 (RAND Corporation), based on the Medical Outcomes Study by Ware, Sherbourne and colleagues.
Your result is a screening aid, not a diagnosis. If you are worried about how you have been feeling, please talk to a health professional.
